After reviewing the Concious Sedation learning module, take the mandatory post-test by CLICKING HERE. If you have trouble accessing the test, please click on the link below for the "Conscious Sedation Post-Test". You must achieve a passing grade of at least 9/10 to pass. You may retake the test once. After you pass the test Click here to access the Critical Elements Checklist for Conscious Sedation. Print (2) copies, fill in your name and signature then give it your PCUM or APCUM to be filed for your record. Retain the second copy for your personal records.
